This information is needed for the Oregon Death Certificate. We will initate the death certificate and deliver to the Doctors office. Once the Doctor completes the medical information, we will retrieve the certificate from the Doctor and file it in the County of death. The State of Oregon charges $ 20.00 for the first copy and $ 15.00 for each additional copy. The State also has a filing fee of $ 20.00.
